Patterns of Prescription of Atypical Antipsychotics in Elderly Ambulatory Patients with Dementia in Korea

Abstract
Objective: To describe the use of atypical antipsychotics (AAPs) in elderly patients visiting ambulatory care with dementia in Korea.

Methods: Outpatients aged 65 or older, with diagnosis of dementia (ICD10, F00-03, G30, G31.8) were included from the Korean Health Insurance Review & Assessment Service database between January 2005 and June 2006. Prescription patterns of AAPs including time trend, frequency and proportion by generic name, type of dementia, type of medical service and regional area were analyzed. Chi-square test was used to compare prescribing patterns between regions and medical services.

Results: The number of patients who received at least one AAP prescription was 15,441 among 111,366 patients with dementia. Proportion of patients treated with AAPs was higher in males (15.1%) and age group of over 85 years (18.1%). In Lewy body and vascular dementia, in which prescription of AAPs is highly restricted, 9.0% and 1.1% received these agents, respectively. There was difference in regional areas along with types of medical institution in the proportion of dementia patients treated with AAPs (p£¼0.01). For all AAPs prescription, risperidone occupied 60.9%, in contrast to amisulpride, zotepine and ziprasidone, which were comparatively rare.

Conclusion: Prevalence of elderly dementia outpatients treated with AAPs was not rare. In light of recent safety-related warnings, caution should be exercised in treatment with AAPs in dementia patients. Effort in monitoring patients receiving AAPs will be needed, in which will provide evidence to issue up-to-date prescription guidelines for elderly patients with dementia in Korea.
